Hi Operation Mosquito G.R.I.D. Agent,

This is your first official communication from Headquarters. Watch the introduction and "Step 1" of the video (1:48 minutes) for instructions and follow the "Parts" below to complete Step 1.

Part 1:
Have an adult complete the Parent Permission Form

Your parent permission form MUST be submitted by the deadline or you will not be able to participate. If your teacher provided you with a hard copy of the parent permission form, turn that form by the deadline.

Part 2:
Pre-assessment

Once you have received permission to participate, fill out all 26 questions of the form below.

Incomplete or late forms will not be accepted and you will not be able to participate in the project.

Be sure to hit "Submit" after you answer all the questions and before you close the page.

Deadline on checklist

Parts 1 - 2 must be completed in order to participate in Operation Mosquito G.R.I.D.

Operation Mosquito G.R.I.D. will require you to place a soda-sized, detection cup somewhere outdoors, either in your yard or on your patio or balcony to capture Aedes mosquito eggs.
